About the Certificate Program

In pursuit of its mission to improve human health by extending the frontiers of clinical pharmacy, the American College of Clinical Pharmacy (ACCP) is offering a certificate program for international pharmacists seeking to advance the outcomes associated with clinical pharmacy practice.

Medication Optimization Through the Provision of Comprehensive Medication Management is a 10-week virtual certificate program. Designed and delivered by subject matter experts, the program will provide evidence-based, implementation-focused programming.

The certificate program will offer five (5) learning modules. Each module will have a pre-session learning and self-assessment component, which includes required readings, pre-recorded video presentations, downloadable slide decks, supplementary tools, and self-assessment questions. In addition, practice experiences will be used throughout the learning modules to help participants apply the knowledge and skills in their own context. Each module will include a live session that will engage participants in dialogue and active learning to prepare for implementing and evaluating medication optimization and comprehensive medication management services at their local practice sites.

Target Audience

The target audience is international pharmacists interested in learning about, implementing, or advancing comprehensive medication management in their practice.

Learning Objectives

Upon completion of the program, participants will be able to:

  1. Examine comprehensive medication management (CMM) and its core components
  2. Relate the essential functions and operational definitions of the CMM patient care process to their own practices
  3. Apply the CMM patient care process to different patient scenarios
  4. Describe the components of the Active Implementation Frameworks and how they are used to guide CMM implementation and evaluation
  5. Determine the parameters necessary to evaluate the implementation of CMM and associated outcomes

Learning Modules

The certificate program will include the following five modules:

  • Module 2. The Patient Care Process for Delivering Comprehensive Medication Management
  • Module 3. Comprehensive Medication Management: Implementation
  • Module 4. Comprehensive Medication Management: Evaluation
  • Module 5. Practice-Based Considerations and Challenges: Putting It All Together

Module Learning Objectives

Module 1. Medication Optimization and Comprehensive Medication Management: A Primer

Upon completion of this module, participants will be able to:

  1. Describe the importance of comprehensive medication management (CMM), including its role in addressing the quadruple aim and medication safety
  2. Explain the tenets of the philosophy of practice
  3. Assess the core components of CMM in one's current practice
  4. Formulate a description of philosophy of practice "in action"
  5. Evaluate how a philosophy of practice “in action” relates to the five core tenets

Module 2. The Patient Care Process for Delivering Comprehensive Medication Management

Upon completion of this module, participants will be able to:

  1. Describe the essential functions and operational definitions of the CMM patient care process
  2. Describe the ten categories of medication therapy problems
  3. Discuss the significance of the patient’s medication experience and how this information is used to guide medication therapy decisions
  4. Compare and contrast one’s current process with the CMM patient care process
  5. Categorize medication therapy problems based on clinical scenarios
  6. Assess patient-specific data and interpret these findings to identify medication therapy problems
  7. Establish goals of therapy for each medical condition and medication therapy problem identified
  8. Formulate recommendations for resolving medication therapy problems, providing rationale, justification, and evidence
  9. Create a plan for patient follow-up

Module 3. Comprehensive Medication Management: Implementation

Upon completion of this module, participants will be able to:

  1. Explain the purpose of practice management
  2. Define the framework for CMM practice management
  3. Examine the five core components of active implementation frameworks
  4. Evaluate the steps required to successfully implement CMM in one’s practice
  5. Describe the basic principles and purpose of documentation of clinical practice
  6. Determine the steps for building relationships with decision makers and other health care team members
  7. Complete a needs assessment/SWOT analysis for building a CMM practice

Module 4. Comprehensive Medication Management: Evaluation

Upon completion of this module, participants will be able to:

  1. Describe how quality improvement can address the quadruple aim
  2. Determine the parameters necessary to evaluate the implementation of CMM and its outcomes
  3. Evaluate ways to create sustainability of a CMM practice
  4. Analyze a practice using a practice management tool
  5. Design a plan for maintaining professional competence and quality for a CMM practice

Module 5. Practice-Based Considerations and Challenges: Putting It All Together

Upon completion of this module, participants will be able to:

  1. Describe a variety of CMM practices
  2. Outline challenges to successfully implement and evaluate CMM practices
  3. Suggest next steps to successfully implement and evaluate CMM practices

Program Faculty

Keri D. Hager, Pharm.D. Keri D. Hager, Pharm.D., BCACP
Keri Hager is Associate Professor in Department of Pharmacy Practice and Pharmaceutical Sciences and Co-Associate Dean for Clinical Affairs at the University of Minnesota (UMN) College of Pharmacy, where she focuses on preparing the future workforce for collaborative team-based patient-centered primary care. Dr. Hager teaches pharmaceutical care practice and facilitates several UMN core interprofessional activities from introductory learners through to post-graduate level learners. Her current focus is on team collaboration in the primary care clinical learning environment to improve mental health and substance use disorder (SUD) outcomes through bridging primary care, mental health, and SUD treatment services. Dr. Hager is currently developing an interprofessional practice connecting patients with SUDs, particularly opioid use disorder, with comprehensive medication management and primary care.
Keri D. Hager, Pharm.D. Jody L. Lounsbery, Pharm.D., BCPS
Jody Lounsbery is Associate Professor in the Department of Pharmaceutical Care & Health Systems in the College of Pharmacy and in the Department of Family Medicine and Community Health in the Medical School at the University of Minnesota. Dr. Lounsbery is also Assistant Director for the Postgraduate Pharmacy Residency Program. She serves on the faculty for the University of Minnesota North Memorial Family Medicine Residency Program and practices at the program’s clinic site. Dr. Lounsbery’s clinical interests include defining the pharmacist’s role as a clinical educator and faculty in family medicine residency programs, implementing an interprofessional curriculum for learners, and providing patient-centered care to underserved patients through comprehensive medication management.
Keri D. Hager, Pharm.D. Deborah L. Pestka Pharm.D., Ph.D

Deborah Pestka is a pharmacist and health services researcher at the University of Minnesota College of Pharmacy, where she also earned her PharmD and PhD. She has done extensive research examining pharmacists’ roles in the delivery of patient care and the outcomes of pharmacists delivering comprehensive medication management. She is currently a Learning Health System Scholar where she is evaluating a population health focused primary care transformation and the implementation of comprehensive medication management into this new model of care. In addition, Dr. Pestka continues to work as a pharmacist in the community setting.

Continuing Pharmacy Education Credit

ACPE The American College of Clinical Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education (CPE).


ACCP designates this activity for 17.25 contact hours. Partial credit is not available.

To earn CPE credit for this certificate program, the participant must successfully complete all required activities for the five (5) learning modules. Participants must (1) complete all required readings and achieve a passing score of 70% or greater on each of the module self-assessment tests (formative) before the relevant virtual session; (2) successfully submit the practice-based activities for each of the modules before the relevant virtual session; (3) complete the associated evaluations for each of the modules; and (4) achieve a passing score of 70% or greater on the final comprehensive test (summative) for the certificate program. All program requirements and the final test must be successfully submitted by October 12, 2021.

To obtain a statement of credit for the certificate program, international participants must update their ACCP profile at www.accp.com/myaccount/licensure.aspx by checking the box for “I am an international pharmacist or I am not licensed to practice in the United States, and do not have an NABP e-Profile ID.” Participants with an NABP e-Profile ID on file with ACCP will obtain their statement of credit from www.mycpemonitor.net.

Certificates of Completion

Certificates of Completion will be issued to participants successfully meeting all program requirements. Certificates will be issued electronically within 8 weeks of program completion.

Registration fees for the certificate program are based on the World Bank classifications of world economies:

  • Low income: $75*
  • Middle income: $110*
  • High income: $150*

Cancellation Policy

  • Requests for cancellation must be made in writing and faxed to ACCP, fax: (913) 492-0088, or by email to [email protected].
  • An administrative fee of $50 will be charged for registrations cancelled before August 1, 2021.
  • Registration fees cannot be refunded for cancellations received on or after August 1, 2021.

Hardware and Software Requirements

Computer and internet access is required to complete the certificate program. Program materials will be available in various formats:

  1. Adobe PDF files. PDF files can be viewed on any desktop/laptop computer with Adobe Reader installed.
  2. MP3 audio files. MP3 format is compatible with most portable audio players, smart phones, tablets, desktop/laptop audio software, and certain car stereos. Information on accessing MP3 files is available at www.accp.com/store/audio.aspx.
  3. MP4 video files. MP4 is a standardized file format supported by almost all video players, including the default video players for Windows and macOS. Android and iPhone also support MP4 files.

System requirements to participate in the virtual sessions using Zoom Meetings are:

  • An internet connection – broadband wired or wireless (3G or 4G/LTE)
  • Speakers and a microphone – built-in, USB plug-in, or wireless Bluetooth
  • A webcam or HD webcam - built-in, USB plug-in, or:
    • An HD cam or HD camcorder with a video-capture card
    • Virtual camera software for use with broadcasting software like OBS or IP cameras
