This session will focus on the utilization of chemoprophylaxis and antithrombotic therapy as it pertains to perioperative patients and their comorbidities and medical history. The session will describe venous thromboembolism prophylaxis burden, prophylaxis modalities, and management strategies in periprocedural, trauma, and patient history (such as bleeding and tranexamic acid use). Additionally, implementation of pharmacist-led chemoprophylaxis consult services will be reviewed including literature review, guideline updates, anti-Xa guided dosing, and hemostatic and blood product stewardship. The target audience will be pharmacists and pharmacy leaders who work in hospitals with operating rooms and trauma services in addition to ambulatory care and community pharmacists who manage anticoagulation for the perioperative patient.
