This learning activity will cover various opportunities for deprescribing in solid organ transplant (SOT) recipients and modulation of medication regimens in this setting.  First, optimization of medication regimens in SOT patients with chronic kidney disease (CKD) will be reviewed. This is imperative in kidney transplant recipients. Additionally, chronic kidney disease remains one of the leading co-morbidities in non-kidney transplant recipients. The activity will discuss optimization of medication regimens to halt progression of CKD and ensure medication regimens are optimized. Second, this learning activity will provide updates for opioid minimizations in SOT recipients. Lastly, it will help pharmacists formulate strategies for management of co-morbidities and medication adjustments in geriatric SOT recipients.
