Technological advances continue to provide innovative ways to personalize medication therapy across therapeutic areas in both the inpatient and outpatient settings. Using one longitudinal patient case example, this session will illustrate opportunities for the incorporation of both pharmacogenomic and pharmacokinetic information to guide therapy recommendations in both the community/outpatient and inpatient settings. Focus will be given to novel applications in these settings, including large panel germline pharmacogenomic testing initiated in the community setting, novel antimicrobial PK-guided therapy, and the role of tumor genomic testing in standard cancer management.
