Transitions of Care Through a New Lens: Advancement of Pharmacists’ and Technicians’ Roles in the Care Transition Spectrum

Transitions of care encompass a multitude of patient shifts across the care continuum – outpatient to inpatient, inpatient to outpatient, unit transfers, etc. Pharmacists play a valuable role in each of these care transitions to allow for pointed, comprehensive medication management. Additionally, pharmacy learners and technicians can assist with transitions of care efforts via medication history attainment, test claim analysis (for discharge medications), concierge discharge medication delivery (meds-to-beds service), and beyond. This session will explore the role of the pharmacist in care transitions, with a focus on implementing services, incorporating pharmacy extenders, and revenue generation in addition to offering contemporary solutions for medication optimization challenges. This session will include three mini-sessions focusing on different areas of opportunity for pharmacist involvement in transitions of care: hospital discharge, ambulatory clinic, and community pharmacy. Each speaker will address transitions of care for the same patient moving throughout the continuum. The session will include active learning activities intended to spark discussion among attendees to identify opportunities and challenges within their own institutions.
