Older adults and those with serious illness experience the highest rate of drug-induced morbidity and mortality due to the high number of medications used, many of which are continued indefinitely and are often inappropriate or unnecessary due to changes in functional status or goals of care. This population has the greatest need for deprescribing; however, the current practice of deprescribing is limited for many reasons, such as opportunity awareness, fear of recurrent episodes, and lack of reimbursement for services. This case-based session will provide attendees with the knowledge, skills, and attitudes to develop, implement, evaluate, and sustain deprescribing into their practice. Key concepts will include how to identify deprescribing-eligible medications, engage patients and their caregivers/families, construct a deprescribing plan, and develop a business plan around deprescribing.
This activity is approved for Board Certified Geriatric Pharmacist (BCGP) recertification credit.
The BCGP Clinical Sessions are part of the professional development program for the recertification of board-certified Geriatric pharmacists by the Board of Pharmacy Specialties and jointly provided by ACCP and the American Society of Health-System Pharmacists (ASHP). In order to earn the BCGP recertification credit, participants must attend the session, claim the continuing pharmacy education credit, and pass the associated posttest. For participants who have not purchased access to the posttest, access can be purchased until 5:30 p.m. on Tuesday, October 15, at the ACCP Registration Desk.
