GTMRx Releases CMM Tools for ACOs and Risk-Bearing Entities

The Get the Medications Right (GTMRx) Institute has released a series of tools designed to provide information about comprehensive medication management (CMM) services to leaders of accountable care organizations (ACOs) and risk-bearing entities. The newly released resources were developed by the Best Practices and Innovative Solutions (BPIS) subgroup of the Practice and Care Delivery Transformation Workgroup – a diverse, interprofessional group of clinicians charged with developing tools to engage and educate key stakeholders to gain support for a standardized definition and process for CMM. ACCP members are prominently represented in the BPIS group. In addition to over 15 successful Medication Optimization Use Cases, this subgroup has developed several marketing and change-package tools targeting self-insured employer, patient/consumer, and physician stakeholders. New tools within the ACO and Risk-Bearing Entities toolkit include:

  • An ACO infographic illustrating how CMM can serve as a value-based solution to optimizing patient health outcomes and reducing total costs of care. The graphical representation includes evidence of CMM’s positive impact on all aspects of the Quintuple Aim and focuses on data to support a positive return-on-investment. For leaders interested in further exploration, a list of action-oriented steps to implement CMM services is included. This leave-behind piece is appropriate for a CMM-naive leadership team and is written for both clinicians and nonclinicians.
  • A set of ACO FAQs informed by input from several ACO leaders. Presented in a Q&A format, this provides information about the evidence of CMM’s impact on all aspects of the Quintuple Aim, describes how CMM is implemented and delivered, and focuses on why an investment in CMM is a high priority for risk-bearing entities.

The BPIS workgroup is one component of GTMRx’s strategy to disseminate the value of CMM to a wide range of stakeholders within health care. Other GTMRx workgroups include Payment and Policy Solutions, Precision Medicine Enablement via Advanced Diagnostics, Health IT to Support Optimized Medication Use, and Practice and Care Delivery Transformation Evidence Based Resources.
