The 2023 ACCP/ASHP BCGP Clinical Sessions are part of the professional development programs for recertification of the Board Certified Geriatric Pharmacist and/or the Board Certified Ambulatory Care Pharmacist approved by the Board of Pharmacy Specialties (BPS) and jointly provided by ACCP and the American Society of Health-System Pharmacists (ASHP).
This educational series explores cutting-edge, contemporary therapeutic topics, and demonstrates scientifically sound clinical reasoning and decision-making. It employs learning activities designed to advance the participants' skills and abilities to critically evaluate the scientific evidence and to effectively incorporate into daily practice significant findings that lead to effective and safe patient care.

The 2023 ACCP/ASHP BCGP Clinical Sessions are offered in four presentations. Each presentation focuses on a different Geriatric Pharmacy/Ambulatory Care Pharmacy topic that is highly relevant for day-to-day practice. Topics include unintentional weight loss in older adults, updates in the management of chronic kidney disease, hypercholesterolemia treatment recommendations, and treating obesity in older adults.

You're Losing It: Better Strategies for Addressing Unintentional Weight Loss in the Older Adult
While a strategy of “find and fix the cause” of unintentional weight loss in the older adult population is ideal, “prescribe the med and move on” is more common in practice, especially because it is often caregivers who are seeking pharmacotherapy for their loved one who is not eating. This interactive, case-based session will explore the nonpharmacologic and pharmacologic contributors to unintentional and problematic weight loss, highlight the evidence behind some of the most common nonpharmacologic and pharmacologic modalities, and provide strategies for pharmacists take back to their practice to ensure appropriate care of these older adults in all care settings.

Filtering Out the Old, and Reabsorbing the New: Updates in the Management of Chronic Kidney Disease
This session will provide attendees with practical information on how to individualize drug therapies for patients living with chronic kidney disease. The landscape of diagnosis and management of CKD is continually evolving, with evidence-based recommendations changing dramatically in the past few years. This session will provide an overview of these crucial and timely updates with an opportunity for attendees to apply the evidence to simulated patient cases while focusing on overcoming therapeutic inertia and advocating for equitable care.
BCACP/BCGP Clinical Session
The Cholesterol Limbo: How Low Should You Go?
This session will focus on the hypercholesterolemia treatment recommendations and recent literature supporting the role of statin and nonstatin therapy. Faculty will review clinical trial data, especially with newer agents, as well as discuss the role of pharmacists in hypercholesterolemia management. Many clinicians follow the 2018 ACC-AHA cholesterol guideline recommendations as the “gold standard” evidence-based guideline by implementing a fixed-dose, intensity-based statin treatment. This may be appropriate for many patients. However, several high-risk patients may require more intense therapy and targeting further reductions in LDL-C with nonstatin therapy including ezetimibe, PCSK9 inhibitors, bempedoic acid, inclisiran, and icosapent ethyl. This presentation highlights how low cholesterol can go safely to reduce cardiovascular risk, especially in older adults. Interactive case studies will be used to facilitate learning.

Weighing In: The Risks and Benefits of Treating Obesity in Adults and Older Adults
Although behavioral interventions are the foundation of weight management in adults, the effectiveness of these strategies may be limited. In older adults, there are limited data demonstrating that lifestyle interventions are best for both safety and efficacy. There are significant concerns that aggressive lifestyle measures for weight loss, especially through calorie restriction without resistance exercises in older adults may result in increased sarcopenia, frailty, functional decline, fractures, and malnutrition. The addition of pharmacologic therapies for long-term management of obesity may help adult patients achieve optimal outcomes. As the medications available for weight management increase, health professionals are faced with options for individualizing treatment regimens. The purpose of this session will be to review management strategies for weight loss, evaluate the latest literature on pharmacotherapy, and apply the guidance to clinical pharmacy practice in adult and older adult populations. Presenters will also discuss special considerations including pharmacokinetic and pharmacodynamic changes, polypharmacy, cognitive and functional status, caregiver involvement and care venues, food insecurity, and socioeconomic challenges for weight management in the older adult. Attendees will participate in case-based learning scenarios via the audience response system and an interactive activity that will increase familiarity with the risks and benefits of anti-obesity medications.

ACPE-accredited providers, like ACCP, are responsible for transmitting this information to NABP to be stored in the CPE Monitor system. The passing point to earn BCGP and BCACP recertification credit is based on an expert analysis of the assessment items in each posttest module. Any posttest submitted before the Specialty Recertification Credit posttest deadline that meets this passing point will earn BCGP and BCACP recertification credits. These credits will be assigned as of the date of test submission.
All participants must have a Board of Pharmacy Specialties (BPS) board certification credential number, BPS ID Number, and a BPS expiration date on file with ACCP for reporting purposes.
